REMINDER

The 2000 series phones have reached their end of life cycle and will be replaced with the 3900 series phones.

 

When will phones be replaced?

1. When a technician is dispatched on a trouble call and it is determined the phone is no longer in working condition.

2. When a phone is being moved to a new location it will be replaced with a comparable phone type.

3. You can also request replacement phones at any time.  Please contact Telecom Administration, x3-3546 to schedule.

 

What type of move will require replacing the existing phone?

Whenever the phone is unplugged from an existing jack and installed at a different jack.

 

Examples of a move.

1. The phone will stay in the same room, but will move from the existing jack to a different jack.

2. The phone will stay in the same building, but move from existing room to a different room.

3. The phone will move from the existing building to a different building.  (Ex: S&T I to JC)

4. The phone will move to a different campus. (Ex: Fairfax to Arlington)

 

What form should I complete? 

Form 21: Request to Move Service.  You will be contacted by a member of Telecom Administration to verify the new phone type(s) that will be installed.  You will also receive an email stating the phone type(s) and cost per phone.  Upon receipt of the email you will need to reply.  This email will be kept on file with the original request.

 

Will my department incur any additional costs?
